Guardian Angel Home Health Care

is comprised of multi-disciplinary  professionals who have expertise in skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services, and home health aide assistance. All services are performed under the direction of your physician and coordinated by our Case Manager. You’ll receive the committed quality service care you need to support your independent living at home.

Our services

  • Skilled assessment and teaching in disease process and management
  • Provide direct care (i.e. catheter care, wound/ostomy care)
  • Medication teaching and administration as ordered by the physician
  • Skilled teaching and training of patient, family, and caregivers to achieve self-care
  • Evaluate for changes in medical condition and report to the physician
  • Spinal Cord Injury Closed Head Injury Ventilator Dependent
  • Bowel & Bladder Training
  • Activities of Daily Living
  • Rehabilitation Clinical Dementia, such as Alzheimer’s
  • Respite Care
  • Diabetic Management
  • Pain Management
  • Nutritional Counseling

Physical Therapy

  • Restore balance, range of motion, strength, and endurance
  • Improve overall functional mobility, ability to walk, and/or ability to get up and down
  • Help determine and assist with obtaining home medical equipment needs
  • Reduce risk of falling by improving balance, strength, and safety awareness

Occupational Therapy

  • Provide recommendations for adaptations to maximize home safety
  • Improve muscle strength, coordination, and endurance required for activities of daily living
  • Establish a routine that allows you to perform activities of daily living without becoming overtaxed
  • Teach patient, family, and/or caregiver in the use of adaptive equipment/device(s) and safety awareness

Speech Therapy

  • Improve the capability to communicate
  • Restore the ability to safely swallow
  • Develop strategies for memory/cognitive deficits

Home Health Aide

  • Provide services under the supervision of clinical staff on a short-term basis
  • Help with personal care needs including bathing, dressing, toileting, transferring, and walking
  • Support patient, family, and/or caregiver with financial and legal concerns (including Advanced Directives, Do Not Resuscitate, Durable Power of Attorney, Living Will, and long-term planning)
  • Provide social and emotional counseling
  • Identify and provide community resources
  • Counseling Emotional Support Coordinate involvement with Community Services and Assist with financial Resources
